- To provide a social work service to adults living in the Bailiwick of Guernsey. To carry out assessments using the agreed standardised format. To participate in co-ordinating services for people wishing to remain within their own homes. To offer support to people who may be moving into care.
- To form relationships with service users to formulate an accurate assessments of complex psychosocial needs of service users, their families and service providers and assess for services in line with local policy.
- To provide complex psychosocial support for service users with terminal illness/life threatening illness and their families, advocating for service users, carers, and families around complex emotional & practical needs.
- To take a key role in responding to referrals to the Adult Social Work Team carrying a caseload of adults in need.
- Assess service users, carers, and family’s psychosocial concerns taking into account cultural, spiritual, psychosocial, social and financial issues and risk evaluation, in partnership with other members of the multi-professional team as appropriate.
- Assess family’s emotional resilience, coping skills, strengths, and difficulties within this practise setting, using expert social work skills & discussion with the multi-professional team.
- Formulate appropriate interventions to meet service user needs in conjunction and liaison with the multi-professional team, the person’s personal networks and other community resources,
- Undertake person-centred assessments, evaluating & reviewing service user and families’ needs regularly and readjust care plans if necessary.
- Attend and contribute to formal multi-professional meetings to review and plan all aspects of service users care as necessary, chairing as appropriate.
- To develop effective links with families and carers (non-statutory & statutory) in order to achieve a comprehensive assessment and to develop an appropriate care plan based on individual need.
- To demonstrate a high standard of practice under their own direction, that reflects a value-based approach to working with adults and families that is person-centred.
- A Social Work Qualification or equivalent.
- Understanding of relevant legislation.
- Relevant Social Work experience.
