Salary: £36,889 - £47,590 per annumSanctuary Personnel is currently seeking an experienced and dedicated Community Mental Health Nurse (CMHN) to join a dynamic team within Adult Community Services. This role offers the opportunity to work within the Duty and Intervention Team, with the potential for progression to Band 6 after a successful 12-month probationary period.Job Summary:The CMHN will collaborate with a multi-disciplinary team to deliver comprehensive mental health care to individuals within the community. This involves adhering to a care model that emphasises teamwork and cooperation with various mental health services. The successful candidate will demonstrate proficiency in supporting individuals with both common and enduring mental health issues, applying principles of the recovery model in their practice. Responsibilities include managing patient care, mentoring junior staff, and adhering to established policies and guidelines.Main Duties and Responsibilities:
- Conduct assessments and develop care plans for patients with complex needs.
- Provide guidance and support to staff in care planning and implementation.
- Initiate risk assessments and take necessary actions to mitigate risks to patients.
- Lead the team during emergencies, ensuring patient needs are met.
- Communicate effectively with multidisciplinary teams and other involved agencies.
- Manage caseloads, coordinate care, and conduct risk assessments in community settings.
- Collaborate with various healthcare professionals and attend relevant meetings and conferences.
- Supervise and mentor junior staff and students.
- Maintain accurate records and adhere to professional guidelines and standards.
- Registered Mental Health Nurse.
- Degree in a related field.
- Portfolio evidence meeting Band 6 competence criteria.
- Evidence of CPD at diploma level.
- Experience working with diverse professionals and ability to work autonomously.
- Strong communication and interpersonal skills.
- Enthusiasm, motivation, and initiative for the role.
- Flexibility to work out of hours.
- Excellent timekeeping and time management skills.
- Commitment to accurate record keeping.
- Proficiency in IT skills.
- Current driving licence.
