Service AreaGrade: - EGI - £40,878 - £44,132 pro rata per annumPart TimeThe States of Guernsey are looking for a Social Work Assistant - CDS Early Years to join the team at Children and Family Community Services.To provide a coordinated response to children and young people with disabilities and complex / additional needs aged 18 months to 4 years and their families within the Child Development Service Early Years Team. Within this role you will contribute to the multi-agency Developmental Concerns Pathways and Review meetings to undertake Early Years Assessments, Coordinate Packages of Care & Support, and ensure that you work in partnership with children, young people and their families to meet their assessed needs within community settings.
