Duties:
  • Be responsible for managing a caseload of patients with heart failure, both newly diagnosed and following discharge from hospital
  • Liaise with other members of the heart failure management team in primary and secondary care to deliver evidence-based care to patients with heart failure
  • Co-ordinate an education programme for community health professionals and will also be a resource for advice
  • Work collaboratively within multi-disciplinary teams and in partnership with other services to provide seamless care
  • Work as part of the team and contribute to its success by being an effective role model and supervising other staff i.e. nurses, healthcare assistants/support workers and students to ensure they are working to the standards that the people using our services deserve
Requirements for the ODP role:
  • Degree or equivalent in Nursing (RGN qualified) with active NMC Registration (Adult)
  • Hold relevant post registration course
  • Be able to prescribe and review medication as a non-medical prescriber or under Patient Group directives
  • Ability to manage and support the department in a critical incident
  • Post Graduate degree or specialist cardiac qualification e.g. ENB 254, 129, 249 or equivalent
  • V300 Non-medical prescriber or willing to undertake course
