Be responsible for organising your own workload in accordance with agreed priorities, policies, and procedures.
- Carry out assessments, develop client-centred treatment programmes with clearly defined goals, and continually evaluate and adapt these as required.
- Facilitate the safe discharge of individuals, ensuring they can transition smoothly to community living.
- Communicate effectively with clients and their carers, as well as other professionals, to deliver comprehensive and integrated care.
- Prepare and present professional reports, participate in case conferences, and engage in community care-planning meetings to support client needs.
- Formulate requirements for the provision of assistive technology and medical devices, ensuring these meet the identified needs.
- Participate in supervision and development activities to continually improve service delivery and client outcomes.
- Contribute to team and service improvement initiatives, and engage in the professional development of peers and the education of students.Requirements:- HCPC registered
- Minimum of 2 years of experience
- Previous experience in wheelchair services
