Social Worker – Adult Integrated Community Care Team

• Deliver a high-quality casework service, providing consultation, support, and specialist advice within the integrated services.
• Act in an advisory capacity on social care issues, educating and mentoring team members as needed.
• Collaborate closely with colleagues across disciplines, including geriatricians, psychiatrists, and community health leads, to deliver comprehensive care plans.
• Conduct assessments using standardised formats and contribute to holistic evaluations with a multi-professional team.
• Offer complex psychosocial support, advocate for service users, and provide specialist social work advice.
• Facilitate and participate in meetings, case conferences, and training to share insights and develop practice.

• Recognised Social Work qualification, such as a Diploma or Degree in Social Work.
• Proven experience working in integrated adult community services.
• Full and valid driving licence.
• Enthusiastic and adaptable, with excellent interpersonal skills and experience in multidisciplinary teams.
• Capable of managing a complex caseload and adept at problem-solving and decision-making.
