- Manage a defined caseload, using evidence-based and client-centred principles to assess, plan, implement, and evaluate interventions and care plans.
- Provide leadership and support to junior staff and students, fostering a nurturing and educational environment.
- Contribute your specialist skills in Learning Disability Nursing to the Multi-Disciplinary Team.
- Undertake the assessment of service users with complex needs and develop, implement, and evaluate programmes of care to meet these needs.
- Guide, advise, and direct other staff in the comprehensive assessment, planning, implementation, and evaluation of care received by service users.
- Conduct risk assessment processes to determine and mitigate risks to the health and well-being of service users with complex care needs.
- Lead a staff team during emergency situations, ensuring the well-being of service users and support for significant others.
- Utilise diverse information and data sources to identify service users’ needs and guide decision-making processes effectively.
- Take part in service development and evaluate the Therapy & Enablement service within HSC as appropriate.Requirements:
- Registered with the NMC.
- Proven evidence of Continued Professional Development (CPD) at diploma level.
- Possess a teaching or mentoring qualification.
- Hold a diploma in a relevant field.
- Level 3 Certificate in Assessing Vocational Achievement or equivalent.
- Strong IT skills.
- Excellent verbal and written communication abilities.
- Possession of a valid driving licence, potentially negotiable with the manager depending on the area.
- Strong leadership and management skills.
- A caring and empathetic nature.
